Once again, we are pleased to have Smile Illinois-Mobile Dentists return to District 54. The dental clinic for Keller Jr. High students will be held at Eisenhower Junior High– 800 West Hassell Road,  Schaumburg, IL 60169 Friday, January 23, 2026.  The Dental team will provide exams, teeth cleaning, fluoride treatment, and sealants. 

**Parents must provide their own transportation for their child to the Eisenhower clinic.

If you are interested in having your child participate in the dental clinic, click the following link and complete the online form. Completed registration is due no later than January 13, 2026.  No paper forms will be sent home. Here is the online registration link:

Steps to take for completing on line form:

1. Select language preference at the top of website next to phone number
2. Enter Keller Junior High zip code: 60194
3. Scroll down and select Helen Keller Junior High from list
4. Complete form and follow on line steps.
5. Submit when complete

NOTE: The State of Illinois requires all students entering 6^th grade and those entering 9^th grade to provide proof that they have seen the dentist within the past 18 months. If the Smile Illinois dentist examines your child, you have fulfilled this obligation. If you are taking your student to your family dentist instead, please have them complete the state required form and return to the school.
